You open the client's card, rub a finger across the body chart, and a pin drops where you lift it; tap the pin to set a finding type, an intensity, and a one-line note. The four SOAP fields, Subjective, Objective, Assessment, and Plan, can stay empty until you have time, and a session can be saved with a chart alone. When the client comes back, their card shows the last session and a cumulative chart with every mark stacked. Records stay on the phone.

Rub the chart, a note pins there

The session screen is a body chart, front and back. Drag a finger over the areas you worked and they warm up under your touch. Lift your finger and a pin lands on that spot. You do not pick a region from a list; you touch the place you actually treated.

SOAP in seconds between clients

Below the chart sit the four SOAP fields. Start from a copy of the last session so the plan carries forward, and drop in phrases you use all the time, such as tenderness reduced or ROM improved, with one tap. Before and after photos attach to the session. Every field is optional; a chart alone saves.

See where they always hold tension

A returning client's card opens with the last session at the top and the cumulative body chart right below it. Marks from every session stack, so the spots you flag most often show darkest. Tap any region and its history unfolds session by session: when it was marked, what type, what intensity, and whether it came down or went up since last time.

What a pin holds

Each pin carries a finding type such as hypertonicity, tenderness, trigger point, or adhesion; an intensity of mild, moderate, or severe; and a one-line note. That is enough to compare a region across visits, and the region history does exactly that, showing the intensities you recorded, as is. It does not assess anyone's condition; it compares your own entries.

Client cards, not a practice management suite

SoapDeck has no booking, no payments, no intake forms, and no client portal. A client card is a name, the sessions, and the cumulative chart. That is deliberate: the alternatives in this space are scheduling and billing suites priced per month, and a therapist who only wants to remember whether the left trapezius was looser than last week does not need one.

A fictional demo client is waiting on the first screen, so you can rub the chart and open a region history the day you install. Deleting the demo takes one tap.

Client records stay on this phone

SoapDeck never connects to the internet. There is no account, no server, and no analytics. The only way anything leaves the phone is when you tap export yourself, either a session as a PDF or the whole database as a backup file. It is free, and it is not a subscription.

Sessions as a stack

Inside a client card, sessions are listed with their date, modality, and how many photos were attached. Opening one shows the chart, the pins, the SOAP fields, and the photos. Copying the previous session is the fastest way to start the next, because most of what changes between visits is the intensity on the same few pins.

Frequently asked questions

Do I have to fill in all four SOAP fields?

No. All four can stay empty. A body chart with pins is enough to save a session, and you can add the words later.

Can I attach photos?

Yes. Before and after photos attach to a session from the camera or the library.

Does it schedule appointments or take payments?

No. It is session notes and client cards only.

Where are my client records stored?

On your phone only. Nothing is uploaded. Export a session PDF or a backup file when you choose to; the backup file holds client records but not photos.

Is there a demo I can try?

Yes. A fictional client with sessions is on the first screen. Delete it with one tap when you are done.

Is it a subscription?

No. The app is free with no ads and no subscription.
