Lash Client Forms is a printable paperwork bundle for lash extension artists who still hand clients a clipboard. It contains four documents: a lash map with a closed-eye diagram and nine numbered zones per eye, a client consent form, a client record card, and a sheet of four cut-out aftercare cards. The aftercare sheet prints four identical cards per page with crop marks, so one sheet through the trimmer gives you cards for four clients. Each card covers the first 24 to 48 hours, oil-free products, daily cleansing and brushing, and refill timing, with a line for the next appointment date.
The set uses a serif heading face, a clean sans body, thin rules, and a single rose-gold accent. There is no logo and no template branding. Every document has a Salon/Artist line at the top, so the paperwork carries your name from the first print without any editing.
