Map the set, zone by zone

The lash map shows both eyes closed with nine numbered zones each. Under the diagram, one row per zone records curl, length, and thickness, and there is room for two sessions on a single page. Style checkboxes cover Classic, Hybrid, Volume, Full Set, and Fill.

Get consent on one page

The consent form walks through the procedure, four risk points including delayed reactions up to 72 hours, a patch test performed or declined choice, an aftercare commitment, an optional photo release with its own decline line, and a release with signature lines.

Keep the client's history

The record card holds contact details, allergies and patch test history, previous work, refill cadence, the glue, curl, length, and thickness you used, and six visit-history rows so the next appointment starts from what worked last time.

What is in the bundle

You receive two ZIP files, one for A4 paper and one for US Letter. Each ZIP contains four single-page PDFs: the lash map, the client consent form, the client record card, and the aftercare card sheet. Nothing needs to be assembled and nothing is locked behind an editor. Open the PDF, print it, and hand it over.

The aftercare sheet prints four identical cards per page with crop marks, so one sheet through the trimmer gives you cards for four clients. Each card covers the first 24 to 48 hours, oil-free products, daily cleansing and brushing, and refill timing, with a line for the next appointment date.

The set uses a serif heading face, a clean sans body, thin rules, and a single rose-gold accent. There is no logo and no template branding. Every document has a Salon/Artist line at the top, so the paperwork carries your name from the first print without any editing.

Why paper still wins at the lash bed

A lash map is filled in with gloved hands while the client's eyes are closed and the tweezers are in the other hand. A phone form does not survive that. Paper does, and the record card means the map from the last set is in the folder when the client comes back for a fill. The consent form gives you a signed page for your own records, which matters far more on the day something goes wrong than on the day everything goes right.
